An SMS can be created in CRM just like an Email or a Phone Call from the Activities menu, or it can be created via a workflow. Each SMS activity can be sent to multiple recipients at a time, to any combination of Contacts, Leads, and Users.
Automation You can create and send an SMS as a part of a workflow, so for example you could create a SMS that is sent to a contact an hour before an appointment is due or a reminder to pay an invoice that is a week late.
